Why aren't the Hard, Very Hard, OVERKILL, and Death Wish difficulty masks unlocking for me?

Unlike the PC version of Payday 2, masks in the console version unlock like any other mask in the game, by selecting a card and drawing one after each heist.  This means that you can potentially score these masks earlier in the game if you're lucky!
